This is not a claim form for compensation. It is a registration form for use by our legal team. We collect the information in this form to assist us in moving the case forward, and so that we can provide you with news and updates about the lawsuit.

You are not required to fill out this form to participate in the class action lawsuit. It is completely optional.

If you choose to fill out this form, you are not retaining Waddell Phillips PC to represent you, and you are not a client of our firm. If the court permits the action to proceed as a class action (this is called certification), you may be a class member. You will receive a notice if the action is certified that will explain your rights as a class member.

All of the information submitted in this form will be treated as confidential and solicitor-client privileged. Any information submitted in this form will only be released as required by court order or as you may consent.
